HWPCP app for iPhone and iPad

Victorias Primary Care Partnership (PCP) strategy brings together local Service Providers who work together within voluntary alliances to improve access to services and provide continuity of care for people in their community. PCP partner organisations focus on better coordination among services, management of chronic disease management, integrated prevention and promotion, and strong partnerships.

The Hume Whittlesea PCP (HWPCP) is follows this Victorian Government Strategy.

To build upon this, the HWPCP have launched the HWPCP App to improve communication and sharing of resources, ideas and bring together Quality Managers from across the catchment area to improve services delivered to the population.
